Our transitional living homes (commonly referred to as a live-by) are a stepping stone to independent living. This temporary housing and assistance option aims to prepare for a successful transition into the community. The role of the staff is to help consumers navigate all aspects of life that contribute to living independently and productively. Services are person-centered and can include:
We want to see consumers set and meet goals to improve their self-confidence and self-worth. Consumers will be encouraged to develop and maintain meaningful community participation and maintain or improve their independent functioning, emotional stability, and recovery consistent with their individual abilities.
The benefits of Deer Path's transitional living program include developing independent living skills, building a positive rental history, community interaction/involvement, staff services and support, and mental health management and recovery.
Consumers are responsible for unsupervised housekeeping and budgeting their funds to obtain groceries and other necessities. While this is generally unsupervised, staff come into the facility daily. We are contracted with CCS for services, though the consumer is responsible for the monthly rent.
Wellness Way is located in the city of Mauston and has a three-person capacity.
Stepping Stone is located in the town of Necedah and has a four-person capacity.
