Path Ways is a 4-bed co-ed state-licensed adult family home (AFH). Path Ways is proud to offer individual, private bedrooms plus two common/entertainment rooms and bathrooms. Path Ways provides 24-hour awake supervision and monitoring, as well as resident-specific programming. Path Ways is a small-scale facility located on the edge of the village of Necedah, allowing easy access to local shopping, library, and parks. Depending on the resident's abilities and treatment conditions, Path Ways allows residents to establish more independence in the community and within the home by incorporating monitored, in-direct, supervised passes. Although Path Ways remains a structured and monitored environment, residents are encouraged to develop independent living skills and interact more frequently within the community.
Path Ways has the ability to work closer, 1:1 with our residents due to our 1:4 staff to resident ratio. Path Ways takes pride in our intensive staff training program in accordance with state statutes.
Each resident receives an initial resident specific assessment. After admission, staff conduct a 30-day assessment and then implements a complete and detailed Individual Service Plan incorporating the resident's needs. Each resident is also provided with an Individual Living Skills program, which emphasizes resident activities of daily living to train, enhance, and support the resident's abilities with household chores, personal laundry, room maintenance, meal planning and preparation, social skills, fiscal management, community integration, mental health management, recovery, and constructive activities.
Path Ways provides residents with a weekly activity schedule identifying various offered constructive activities, therapeutic groups, and recreational outings. Path Ways provides referral and support as needed for each resident's specific medical and psychiatric needs. Path Ways also provides a complete and secure record of resident's medication management. Staff document each resident's daily progress to better identify symptoms and areas of concern to obtain services, treat, and implement a record of prompt and adequate treatment.
