Residents who are accepted into the program must be ambulatory, mentally and physically capable of responding to an electronic fire alarm, and exiting the facility without any verbal or physical prompting.
We will not accept any resident needing skilled nursing care or constant hospitalizations or any resident who is bedridden. All residents shall receive an initial health assessment, which will be administered no more than 90 days before or within seven working days after admission. The assessment shall include a TB skin test and documentation of the results at the time of admission. Annual health assessments for dental and vision will be conducted, as well.
We charge for the day of admission, but not the day of discharge. If any refund is due upon discharge, the designated party will receive the refund within 10 days after discharge.
We have the legal guardian and/or designated representative's authorization to obtain emergency health care services at the resident's expense under the following circumstances: any services that are not covered under Medicare, Medicaid, or Wisconsin Medical Assistance.
Upon request, the results of all department license renewal visits, monitoring visits or complaint investigations (within 12 months of the inquiry), will be made available.
Prior to admission or within 5 days after emergency admission, we will determine with whom we will need to communicate regarding the health, safety, welfare, and rights of any resident admitted to the facility and will attempt to obtain the needed releases of information from the resident or resident's guardian.
