Residents accepted into the program must be ambulatory, mentally, and physically capable of responding to an electronic fire alarm and exiting the facility without any verbal or physical prompting.
We will not accept residents needing skilled nursing care, constant hospitalizations, or bedridden care. All residents shall receive an initial health assessment administered no more than 90 days before or within seven working days after admission. The assessment shall include a TB skin test and documentation of the results at admission. Annual health assessments for dental and vision will also be conducted.
We charge for the day of admission but not the day of discharge. If a refund is due upon discharge, the designated party will receive it within ten days after discharge.
We have the legal guardian and/or designated representative's authorization to obtain emergency health care services at the resident's expense under the following circumstances: any services not covered under Medicare, Medicaid, or Wisconsin Medical Assistance.
The results of all department license renewal visits, monitoring visits, or complaint investigations (within 12 months of the inquiry) will be made available upon request.
Before admission or within five days after emergency admission, we will determine with whom we must communicate regarding the health, safety, welfare, and rights of any resident admitted to the facility. We will attempt to obtain the information needed to be released from the resident or the resident's guardian.
